Sutter Health Alliance - Valley
This position is responsible for operationalizing Sutter Health Plan's health-related social needs (HRSN) screening and intervention program in accordance with NCQA's Social Need Screening and Intervention (SNS-E) measure and other contractual requirements. Annually or more frequently, the incumbent screens Plan Members aged 18 and older for food insecurity and additional health-related social needs (e.g., housing instability, homelessness), coordinates screening and closed-loop referral efforts with Participating Providers, links members to appropriate community and social services across the Service Area, and produces the analysis and reporting required to demonstrate compliance to SHP’s client and internal executive leadership. Screening is administered through a blended digital and telephonic outreach model that leverages the member resource directory/community resource platform maintained by SHP's contracted vendor.

Pay Range is $50.83 to $71.15 / hour
The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs.
