Job Details:

The Care Coordinator Social Worker ensures quality patient care in the appropriate setting by assessing key needs, addressing local and systemic issues interfering with care, assisting families in setting appropriate goals and tracking progress toward achieving those goals. Navigates the health care system on behalf of high need patients (including delegates); supervises and provides care coordination; plans for education; and provides for patient safety and quality of care. Serves as an agile problem solver on multiple levels, from patient-level, to disease population and care system. Practices professional social work and promotes patient advocacy in accordance with the NASW Code of Ethics for Social Work.

These activities are performed in a variety of settings including during inpatient stays, outpatient specialty clinic visits, emergency room visits, home, and community visits, as well as telephone and “virtual” visits.

Education

Master’s Degree in Social Work required

Certifications:

LSW Ohio - Ohio Licensed Social Worker - Counselor, Social Worker and Marriage & Family Therapist Board required

BLS required within the first 60 days of hire

Department Specific Job Details:

  • full time, 40 hours
  • Monday - Friday, 8:00 AM - 4:30 PM
  • no weekends or holidays
  • serving the Partners for Kids program at the Tech Town location
