The Behavioral Health Case Manager provides comprehensive support and coordination of care for clients facing mental health challenges. This role involves assessing client needs, developing individualized service plans, and collaborating with multidisciplinary teams to ensure the delivery of effective and client-centered services. The Case Manager is dedicated tofacilitatingclients' recovery and promoting their overall well-being and independence.

Duties and Responsibilities:

  • Coordinate and provide care that is safe,timely, effective, efficient, andclient-centered.
  • Manage case assignments, draft service plans, and review case progress regularly.
  • Advocate for clients,assistingthem in making informed decisionsregardingtheir clinical status and treatment options.
  • Collaborate with therapists, clients, and client family members to ensure all clientbehavioral healthneeds are met.
  • Facilitate clients in achieving personal independence andassistin developing daily living skills.
  • Perform other duties as assigned.
  • Upholds Documentation and Billing practices associated with CARF, CMS, and as trainingindicates.

Supervision Given: 

This position does not supervise any staff.

Supervision Received: 

The Behavioral Health Case Manager reports to their assigned OperationsManagers.

Qualifications:

  • Strong interpersonal skills and the ability to communicate effectively with clients, families, and professionals.
  • Knowledge of behavioral health practices and community resources.
  • Ability to work independently and as part of a team.

Education:

  • High school diploma or equivalent isrequired;
  • a collegedegree in Human Services is preferred.

Certification:

  • CPR certification is preferred.
  • NPI number/Ohio Medicaid ID isrequired(instructions on how to apply will be provided).

Experience:

  • Minimum of3yearsin behavioral health or a related field is preferred, but not mandatory.

Physical Effort:

  • Requires sitting for extended periods and occasional lifting of up to 20 pounds.

Background:

  • Must pass all Federal and State background checks according to company policy.

  • Flexible scheduling options allow for accommodating personal availability, enabling as little or as much work as desired.
