
The Money Follows the Person (MFP) program is a federally funded demonstration project within the Adult Services Division of DAIL that helps older Vermonters and Vermonters with disabilities transition from hospitals and nursing homes to homes in their communities. The MFP project Director administers and monitors the MFP Demonstration Grant and is responsible for staff Supervision. The director participates in collaborative program policy and development within the Adult Services Division, other state agencies, and local services providers. Monitors contracts with community-based organizations and provides program content expertise through technical assistance and training. The director identifies gaps in the Long-Term Care Medicaid system of care and works to address the gaps and improve services. Federal reporting, data analysis, and creativity in developing solutions are every day aspects of this role. Performs other related duties as required.
The successful candidate will have excellent attention to detail while also being able to see the system as a whole, technical writing skills, technical skills with excel and ability to learn new software programs, an understanding of Long Term Care programs, a growth mindset, and a desire to learn. The position is located in Waterbury, and some telework may be an option. This position reports to the Adult Services Division Assistant Director.

Duties are performed primarily in a standard office setting. Meetings with community organizations will require significant travel, for which private means of transportation must be available. Some pressure from deadlines may be anticipated. Some work outside of normal office hours may be required.
Master's degree or higher AND two (2) years or more at a professional level working with services to the elderly and/or disabled.
OR
Bachelor's degree AND four (4) years or more at a professional level working with services to the elderly and/or disabled.
Experience working with home- and community-based programs.
Experience working with Medicaid programs.
Candidates must pass any level of background investigation applicable to the position. AHS Policy 4.02, Hiring Standards, pursuant to 20 V.S.A. 2056c or other applicable statutory authority, requires criminal record checks for all AHS positions, including motor vehicle driving record checks and national record checks where appropriate.
