Job Description

Thank you for expressing your interest in working with Fleet Healthcare (FHC). Prior to you commencing work with us we require the following documents and information.

Below is a list of documents that we require from you to remain compliant with the companies and agencies that support our clients. Items that are asterisked are essential as part of the application process. Remaining items that aren’t asterisked are optional for the application process, but may be required at a later date. Please note, not providing documentation may prohibit you from treating certain clients.

  • Business Structure (Sole Trader / Company / Trust)
  • Superannuation Details (Fund Name, USI, and Membership Number) - only required if operating as a sole trader
  • National Police Certificate

  • First Aid
  • CPR
  • Australian Drivers Licence
  • Passport
  • Working With Children Check
  • NDIS Worker Check (NDISWC)
  • Comprehensive Car Insurance
  • Flu Vaccination Documentation

Payment Information:

Prior to commencing work with Fleet Healthcare we are required to confirm payment arrangements with you. Working with Fleet Healthcare you have the option of being paid the FHC rates or setting your own rates. Please review the below information and advise which option in your application you would like to proceed below:

  • FHC rates: At this link you will find the "FHC Chiropractic Payment List" Providing services at these rates gives you the ability to receive the maximum number of client referrals.
  • Setting your own rates: In the "Application" section of this webpage you will find a field to input your chosen rates for the services you provide. Please note that by setting high rates, Fleet Healthcare may be required to increase the client charge to remain profitable, which may reduce the number of clients agreeable to receiving your services.


Service Area:

With Fleet Healthcare, you have the option to choose your Local Government Areas (LGA) within Sydney. Tailor your service locations to fit your preferences and convenience. Please open this link to reference when selecting your LGA: Sydney LGA Map You will be required to make a selection in the application process.

Availability:

Working with Fleet Healthcare we offer great flexibility in allowing you to determine your own schedule and work when you want.

In the application, you will be asked to select which option you would prefer:

  1. Create set availability with Fleet Healthcare - please advise days / hours
  2. Be contacted when appointments become available in your service area

Our Podiatrists deal with the prevention, diagnosis, treatment and rehabilitation of medical and surgical conditions of the feet and lower limbs. The conditions podiatrists treat include those resulting from bone and joint disorders such as arthritis and soft-tissue and muscular pathologies, as well as neurological and circulatory disease.

Our Dietitians treat a range of health conditions. They understand how nutrition affects the body and will provide expert nutrition and dietary advice.
