Chartiers Center is a private, non-profit corporation funded by state, county, third party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach, and housing program.
As a Mobile Case Manager with Chartiers Center, you would be working in the community to provide support and services to individuals with mental health challenges.
Job duties include:
Coordinate and oversee the delivery of case management services to ensure that the individuals are connected to various benefits and services such as SNAP, CAP, LIHEAP, LIHWAP, SOAR, OP, and MPR.
Assess individual’s needs and develop individualized service plans to meet those needs.
Maintain regular communication with clients to monitor progress and address any concerns.
Collaborate with other service providers and/or administrative staff to ensure seamless delivery of services.
Maintain strong community relationships with contacts for education, employment, housing and natural supports.
Maintain accurate records of interactions, service plans, and progress reports.

Minimum
Qualifications
Bachelor's degree in human services and 2 years of mental health experience OR
Master’s degree in human services with one year of experience
Valid Driver’s License Required
