Mission Transition High School Facilitator
Facilitators will work collaboratively with DAC (Disability Action Center) and SE high school liaison in planning instruction and job outings to facilitate successful high school transition opportunities for students with disabilities.
Schedule and supervise monthly in-class Mission Transition workshops
Schedule and supervise the annual Mission Transition job shadowing field trip
Coordinate the Transition Fair trip with school department chair
Maintain ongoing collaboration with the Mission Transition Job Coach throughout the school year
Monitor and communicate each student's transition and employment goal progress
Submit activity time log at the end of each semester
Rate of pay for each facilitator will be $500.00 per Semester.
