The Physical Therapist provides professional physical therapy services to home health clients in accordance with physician orders, the established plan of care, applicable state practice acts, licensure requirements, and organizational policies. This role performs comprehensive assessments, develops and implements individualized treatment plans, and delivers preventive, therapeutic, and rehabilitative interventions to promote functional independence and optimal outcomes in the home setting.
The Physical Therapist coordinates care with physicians and the interdisciplinary team, communicates changes in patient condition, and ensures accurate and timely clinical documentation. Responsibilities include patient and caregiver education, environmental assessments, discharge planning, quality improvement participation, and supervision of PTAs and Home Health Aides as permitted by state practice acts. The role requires independent clinical judgment, adherence to regulatory and ethical standards, and a commitment to quality, compliance, and patient‑centered care.
