The Clinical Transition and Stabilization Services (CTSS) Mental Health Worker will assist emotionally and behaviorally troubled youth with transitioning into and remaining stable within therapeutic foster homes. Within this innovative clinical support program, all services will be provided under the direction of the clinical team, including CTSS Director and team Therapist.

Rate: $21.00/hour
Travel: Work in schools, homes, and community settings; mileage reimbursement at $0.65 per mile
We are seeking a dedicated Behavioral Health Case Manager to join our team and support children and youth in community-based settings. This position requires strong communication skills, the ability to collaborate effectively with multidisciplinary teams, and comfort navigating multiple systems at once.
Provide behavioral interventions and case management to children and youth
Collaborate with families, schools, and community partners
Maintain clear and professional communication, both orally and in writing
Travel throughout Philadelphia to meet clients where they are—homes, schools, and community spaces
Bachelor’s degree in a mental health–related discipline (social work, psychology, sociology, counseling, criminal justice, education, or related human services field) from a regionally accredited program
At least 1 year of post-graduate experience providing behavioral interventions and case management to children or youth in a mental health or child welfare setting
Ability to work effectively as part of a team and manage involvement across multiple systems
Strong oral and written communication skills
1–2 years of experience working with children or youth in behavioral health
Experience with mental or behavioral health and case management preferred
Comfortable traveling throughout Philadelphia
