POSITION SPECIFICS
Title: Mental Health Therapist
FTE: Full Time (40 hours/week)
Schedule: Monday-Friday days
Holiday Rotation: None

Location: Sauk Prairie School District
The Mental Health Therapist functions as an independent healthcare provider who, working collaboratively within a multidisciplinary team, provides diagnostic assessment and treatment to applicable demographic population to address the needs of these patients and provide clinical mental health services. Responsibilities include assessment/intake, treatment planning, individual clinical therapy, and related documentation. Emphasis is placed on the provision of evidence-based clinical behavioral health practices. These services may be provided in a variety of settings, dependent on the population served (i.e. clinic settings, via telehealth, school settings, senior living facility). In addition, this role may also perform duties associated with the Behavioral Health department in connection with the clinics/hospital as needed.
POSITION TECHNICAL RESPONSIBILITIES
Perform assessments to evaluate a variety of mental health and behavioral conditions. Collaborate and consult with a multidisciplinary team to address the needs of the population served, provide quality patient care, and to promote SPH’s vision and mission. Coordinate patient care with a variety of resources. Resources will vary based on specific population served and could include, but is not limited to: schools, families, primary care, senior living facilities, and other specialty providers as needed. Conduct individual/family/group counseling, utilizing evidence-based practices. Utilize outcome tools to determine patient progress. Assist in contributing to continuity of care planning as patients transition to other community-based services. Maintain progress notes and other required electronic documentation. Serve as a subject matter expert as part of a multidisciplinary team, provide feedback, and consult as needed. Work collaboratively with Sauk Prairie Healthcare on projects and initiatives related to Behavioral Health Services. Provide care within their scope of practice as outlined by applicable state law, licensing, regulations, SPH policy, and practice agreements.
POSITION REQUIREMENTS
Education:
Required: Master’s degree in Social Work, Psychology, or Counseling.
Preferred: None
Experience:
Required: 2 years of experience providing evidenced-based treatment as a mental health provider.
Preferred: 3-5 years of experience providing evidenced-based treatment as a mental health provider to include some experience working with students/families in a school-based setting. Experience working with substance use disorders.
Licenses and Registrations:
Required: Relevant license: Licensed Clinical Social Worker-LCSW, Licensed Professional Counselor-LPC, Licensed Marriage and Family Therapist-LMFT, or Licensed Psychologist.

Certification(s):
Required: Basic Life Support (BLS) – within 3 months of hire
