Schedule: Monday-Friday, 8:30 am-4:30 pm
Earn $19.03/hour or $21.03/hour with the selection of Enhanced Pay, plus mileage reimbursement
Position Details
The Mental Health Case Manager plays a pivotal role in enhancing the mental health of adults, children, and adolescents in Cumberland and Perry Counties They coordinate resources and services, support families, and tailor care to each individual's unique situation. Their work focuses on harnessing individual strengths, connecting with community resources, and promoting stability. While operating independently, they are backed by a dedicated team, all within a dynamic community program. The case manager actively engages with consumers and their families in the home or community setting.
