This position is located at the SC Department of Behavioral Health and Developmental Disabilities (BHDD), 400 Otarre Parkway Cayce, SC 29033.

As the Mental Health Professional III / Transition Specialist Supervisor, under the supervision of the Transition Specialist Program Manager, you will plan, manage, direct, and evaluate the BHDD Transitions Specialist Team to achieve patient discharges from inpatient settings to integrated community residences and community based behavioral health services.

Responsibilities for the Mental Health Professional III / Transition Specialist Supervisor include:

  • Supervise the Transition Specialists in execution of all essential job duties to ensure the team is operating according to regulatory requirements, including but not limited to the oversight of patient services, program evaluation, arid record-keeping.
  • Serve as the liaison to Inpatient Services / inpatient hospitals as it pertains to patient movement, discharge plan development, and directs and manages the workload of the Transition Specialists. Coordinate staffing with Community Mental Health Centers (CMHC) regarding program placement for patient when discharged; provide face-to-face and/or video
  • introduction of patient to community providers to include care coordinator and peer support specialist; attend treatment team meetings, discharge committee meetings, patient staffings and discharge planning staffings to provide community-based resources and/or options for discharge planning.
  • Collect and maintain data and reports to monitor Transition Team productivity and discharge plan development. Prepare and submit data, reports,
  • and documents for leadership. Review and signoff on clinical documentation and paperwork. Identify, collect and routinely update an inventory of integrated community-based residences for Transition Team distribution/resource directory; broker needed community resources/services and develop resources where gaps exist.
  • Identify and coordinate efforts to build statewide partnerships with treatment providers and residential housing resources to develop comprehensive community resources for patients.

Examples of duties

  • A master's degree in social service and four (4) years of post-graduate experience (one (1) year of which must have been in an inpatient and/or community mental health program) or 48 hours towards a doctorate in psychology and three (3) years of post-graduate experience (one (1) year of which must have been in an inpatient and/or community mental health program).

Additional Requirements:

  • Ability to bend, stoop, climb, drive from location to location, lift and file, and perform duties in multiple settings.

Qualifications

  • Bilingual abilities in English and Spanish (or another language) are a plus.

  • Advanced knowledge and experience in assessing and treating adults living with serious mental ilinesses through all stages of recovery.
  • The ability to supervise staff of varying backgrounds, training, and experience and experience in mental health settings.
  • The ability to lead a team of professionals to accomplish program goals.
  • Experience in data-based planning, evaluation, and decision making.
  • The ability to demonstrate exceptional communication skills both oral and written.
