This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, Lexington County Community Mental Health Center, 301 Palmetto Park Blvd, Lexington, SC 29072.

As the Mental Health Professional, working under limited supervision, you will serve as the Center's Housing Coordinator to ensure patient access to appropriate community-based housing.

Responsibilities for the Mental Health Professional/Housing Coordinator:

  • Provide oversight for the Center's housing programs, including Community Housing, Homeshare, Homeshare Respite, TLC/Housing and Homeless Program Supportive Housing developments, Community Residential Care Facilities (CRCFs), and other supportive housing programs, as applicable. Coordinates administrative requirements of the agency's housing programs, including payment contracts and patient agreements, outreach and recruitment efforts for Homeshare providers, and completion of assessments for patients living in CRCFs.
  • Coordinate with the agency's Transition Programs staff, including Clinical Care Coordinators (CCCs) and Transition Specialist, MCO CCCs, Certified Peer Support Specialists, and Benefits/Entitlement Specialists, Partners with the local HUD Continuum of Care, public housing authority(ies), and affordable housing owners and property managers. Coordinates patient housing referrals and move-ins and maintains wait lists for each Center housing program.
  • Enter and maintain Community Housing data in the agency's Electronic Health Record. Prepares and submits the monthly Community Housing report.
  • Enter and maintain Center Homeshare/TLC program and patient information in the agency's SharePoint site and CIS/Electronic Health Record.
  • Participate in other OMH or partner agency programs and activities as may be required. Attends treatment team meetings daily and provides consultation regarding complex cases as appropriate.
  • Complete documentation according to QA/SCDMH standards, Complies with Corporate Compliance Policy and Risk Management Plan, and Ensures timely completion of prior authorizations.
  • Complete all annual required trainings to include Safety, MAPS, Cultural Affairs, and clinical trainings. Demonstrates adherence to CARF standards, and SCMH and Center policies and Procedures.

Examples of duties

  • A master's degree in guidance, counseling, the social sciences.

Additional Requirements

  • Must be licensed or eligible for licensure in SC and willing to work towards licensure in SC within first six (6) months of employment.
  • Ability to lift 5lbs and operate a state vehicle.

Qualifications

Bilingual abilities in English and Spanish (or another language) are a plus.

  • Ability to listen and understand and communicate information and ideas so that others will understand
  • Knowledge of psychopathology, family dynamics, family and group psychotherapy. Ability to identify and effectively utilize resources available in the community to accomplish objectives
  • Ability to establish and maintain effective working relationships with individuals, families and representatives of other professional disciplines
  • Ability to present clear and comprehensive oral and written reports
