Job Description

Keystone Mental Health Services is currently seeking an Intensive Case Manager to join our team of professionals in making a positive difference in the lives of our neighbors and the community.

The Intensive Case Manager supports adults with mental illness by providing education, assistance and resources in effort to improve overall quality of life. This position helps individuals live independently, participate in vocational/educational programs, access medical care and gain adequate social supports.

Minimum Educational Requirements (One of the following)

  • Bachelor’s degree with major course work in sociology, social work, psychology, gerontology, anthropology, criminal justice, theology, nursing counseling or education
  • Valid Registered Nurse license in the state of Pennsylvania
  • Twelve college credits in sociology, social welfare, psychology, gerontology or other social science field with two years of experience in public or private human services; One year in direct care
  • High school diploma or equivalent with 5 years of mental health direct care experience in public or private human services with employment as an intensive case management staff person prior to April 1, 1989

Additional Minimum Requirements:

  • Computer proficiency in MS Word, Excel, PowerPoint and Outlook and able to learn other software as needed
  • Effective communication skills, verbal and written
  • Able to read, interpret and apply policies, plans and regulation
  • Valid driver’s license
  • Daily access to privately maintained and insured vehicle depending on work location
  • Successful completion of the pre-employment process, including motor vehicle record and criminal background checks
