Conduct behavioral health assessments and intake evaluations
Engage with newly referred clients and families within two business days
Develop individualized, person-centered treatment plans
Provide therapeutic interventions, crisis services, and community support
Coordinate care with providers, families, and community resources
Monitor client progress and participate in treatment reviews
Maintain timely, accurate clinical documentation within the electronic health record (EHR)
Master's degree in:
Counseling
Social Work
Psychology
Behavioral Health
Marriage & Family Therapy
Or a closely related human services field
Eligible to obtain or currently hold a provisional clinical license in Illinois
Minimum of one year of experience in the field of behavioral health (case management, direct care, counseling, or related)
Experience working with individuals experiencing mental health and/or substance use concerns
Assessment, crisis intervention, or intake experience
Knowledge of community resources and behavioral healthcare systems
