Connected Care is looking for a Mental Health Clinician to join our team across Southern Tasmania.

About Connected Care

Connected Care is a statewide mental health service delivered by Wellways in partnership with Stride Mental Health and Clarity Health Care and commissioned by Primary Health Tasmania.

Using a “no wrong door” and stepped-care approach, Connected Care supports adults to access the right level of mental health care and support at no cost, when and where they need it.

Where you come in

We’re looking for a qualified and registered clinician with experience supporting people with severe and/or complex mental health needs. You’ll bring sound clinical judgement, strong assessment and therapeutic skills, and the ability to coordinate care across multidisciplinary teams and service systems.

You’ll be committed to trauma-informed, recovery-oriented and culturally safe practice, with the confidence to build effective relationships with participants, families, carers and other professionals.

​You’ll Need:

You’ll need a relevant tertiary qualification and current registration or accreditation in your discipline, along with experience in mental health assessment, therapeutic intervention, care coordination and outcome measurement.

You’ll bring strong relationship-building skills and the ability to work collaboratively with participants, families, carers and multidisciplinary teams, alongside an understanding of person-led practice and the ability to balance duty of care with dignity of risk. A willingness to learn and apply stepped-care assessment tools and an understanding of the Tasmanian mental health system and legislation are desirable.

A current driver’s licence and willingness to travel across SouthernTasmania are essential.

You’ll also need to meet Wellways’ required compliance and screening checks including:

  • Right to work in Australia
  • 100 points of ID
  • Valid Driver Licence and willingness to travel
  • Satisfactory National Police Record Check
  • NDIS Worker Screening Check
  • NDIS Worker Orientation Module Certificate – Quality Safety and You
  • Current Working with Vulnerable People Check
