The Mental Health Case Manager works directly with clients in high-intensity substance abuse treatment, and performs duties relating to case management including making home visits, assisting clients in obtaining supportive services, making necessary referrals and providing training regarding independent living skills. May include group facilitating.
ESSENTIAL FUNCTIONS

The Mental Health Case Manager performs an integral role in carrying out the mission of The Counseling Center, Inc., by performing the following duties:
- Assesses needs of client, assists in connecting to internal and community resources.
- Facilitates services as required by individualized service plans including anger management, parenting, and mandated interventions within the community psychiatric supportive treatment scope of practice.
- Supports implementation of the mental health treatment plan, collaborates with therapy and medical teams, ensuring continuity and quality of care.
SECONDARY FUNCTIONS
Secondary functions are duties, which are not exclusive of the position, can be performed by other positions; however, secondary duties are to be performed for the efficiency of The Counseling Center, Inc.
- Attends weekly staff and clinical meetings to share in problem solving and addressing specific client issues.
- Performs other duties as assigned by the Program Manager/Director.
COMPETENCIES
- Demonstrated knowledge of substance use disorder, other mental health and dual diagnosis, and corresponding treatment services.
- Demonstrates understanding of clinical practice and performance including understanding of the process of assessment, treatment, and confidentiality.
- Ability in effective communication, interpersonal skills.
- Ability to complete all documentation in accordance with agency policy.
- Effective time management, organizational, and prioritization skills.
QualificationsMINIMUM QUALIFICATIONS, INCLUDING TRAINING AND EXPERIENCE
- High School Diploma or equivalent, Behavioral Science Degree preferred.
- Registration as a Qualified Mental Health Specialist (QMHS) with at least 1 year of experience, with 2+ years of experience preferred.
- Knowledge of alcoholism and other addictions, experience with 12-Step Recovery Program preferred.
- Valid Ohio Drivers License and ability to maintain insurability preferred.
- Maintain CPR/First Aid treatment preferred.
- Maintains applicable agency trainings.
