Job Title: MH Associate – PH/Hopewell/ACRM
Reports To: Unit Director
Trainings: Annual, CPI, QRPT, Heart Saver First Aid CPR/AED
Career Development: MHA ? Program Manager ? Therapist-Non LIP-Therapist LIP
The Mental Health Associate assists in the collection of data and the completion of a Master Treatment Plan for all assigned clients.
Duties and Responsibilities:
•Assists in the collection of data for each assigned client.
•Participates with team members and clients/families in the completion of a Master Treatment Plan for each client within established time frames.
•Participates with team members in the updating of Master Treatment Plans within established time frames or as indicated by client needs.
•Effectively facilitates psycho educational groups as evidenced by:
Preparation.
Consumer satisfaction.
Supervisory evaluation.
•Participates with team members in the completion of a Discharge Summary within established time frames.
•Documents all client interactions, including sessions with clients, families, significant others, etc., within established time frames.
•Completes Progress Notes accurately, addressing treatment goals, major treatment issues, and changes in mental health status.
•Writes in a professional and competent manner.
•Develops an adequate professional working relationship with all referral sources and relevant community agencies.
•Assists with case management and interagency coordination of services for clients. Makes appropriate recommendations for referrals.
•Accurately and promptly completes all reports, documentation and records.
•Works well with staff and supervisors.
•Seeks supervision in a professional manner and documents supervision.
•Demonstrates a willingness to serve on professional committees, attend staff meetings, attend in-services, etc.
•Maintains valid NJ driver’s license.
•Effectively facilitates social and recreational activities.
•Assists in the interviewing process for job applicants.
•When appropriate, acts as an advocate on behalf of the client for all services that are not provided directly
•Provides outreach to re-engage clients or link to other services which will meet their needs
•Provides transportation for clients to and from program related activities in a safe and effective manner, as needed.
•Documentation must be completed within one (1) calendar day of service delivery
•Productivity goals shall meet % as designated by department
Minimal Qualifications:
Education: Bachelor’s Degree in behavioral health field CADC or currently tracking
Experience: One (1) year experience with population, or appropriate 1 year internship
Equipment Use: Computer, copier, facsimile, motor vehicle
