Member Navigator (Care Manager)

Department: Admin

Employment Type: Full Time

Location: Grand Rapids, MI

Reporting To: Paige Hendrickson

Compensation: $65,000 - $70,000 / year


Schedule Monday - Friday, 8am-5pm | Rotating on-call schedule during weekend hours.

What You'll Do:
As a Member Navigator, you are at the heart of the Avenues experience, serving as a trusted advisor, advocate, and lifelong partner to our members. You don’t just support individuals; you build meaningful, enduring relationships that empower people to age with confidence, dignity, and independence. Avenues is a membership-based program designed for individuals who wish to age confidently and safely in their own homes. As a Member Navigator, you’ll be part of a forward-thinking team dedicated to elevating the standard of aging well and you’ll play a pivotal role in making that vision a reality.

Key Responsibilities

Member Support

  • Provide personalized, one-on-one guidance to members, assisting them in understanding and accessing available healthcare and wellness services, benefits, and resources.
  • Build strong rapport and therapeutic relationships with members, their families, and other healthcare team members.
  • Clearly communicate members' rights, options, and responsibilities regarding healthcare and wellness.
  • Enhance members’ and families’ health literacy by explaining medications, medical terminology, insurance.
  • Conduct comprehensive assessments to identify members’ short- and long-term needs, developing actionable strategies and goals to address those needs.
  • Oversee and coordinate caregivers and support staff in delivering high-quality care.
  • Regularly monitor members' progress, address concerns, and adjust care plans as necessary.

Other Responsibilities

  • Accurately document member interactions and changing needs in the Electronic Medical Record (EMR) system to inform and coordinate with the care team.
  • Maintain financial accountability in line with organizational standards.
  • Participate in after-hours phone calls and, when needed, conduct after-hours home visits.

Skills, Knowledge & Expertise

  • Experience in healthcare navigation, case management, or as a medical/health professional is strongly preferred.
  • Experience working with the senior population, required.
  • Proficiency in medical terminology, required.
  • A valid driver’s license, reliable transportation, and proof of auto insurance are required.
  • Bachelor’s degree preferred, ideally in a related field.
  • Strong interpersonal and communication skills, including active listening and the ability to show empathy.
  • Demonstrated ability to develop and implement a holistic, wellness-focused approach to health and well-being for members.
