HOACNY provides cancer care services, Monday through Friday, day hours at three convenient locations in East Syracuse, Syracuse and Auburn. Medication Services Nurse Navigator - Monday through Friday (no nights, weekends or holidays)

This individual is responsible for organizing and assisting in the daily activities of the Physician Dispensing department within Hematology-Oncology Associates of CNY, PC. Duties include: dispensing commercially available drugs; assessing medication orders; assists with patient care activities including planning and implementing antineoplastic and related drug regimens; assist in completing necessary prior authorizations, assist with REMs programs, monitors patients’ response to drug therapy by way of new medication start follow up phone call and recommends changes or alternative therapy and provides drug related information. Conduit for triage and communication between the MID and clinical teams. Documents within HOACNY and ACHC standards, policy and procedure.

Knowledge of professional nursing theory and practice to give and evaluate patient care. Knowledge of organizational policies, regulations and procedures to administer patient care. Knowledge of medical equipment and instruments to administer patient care. Knowledge of common safety hazards and precautions to establish a safe work environment. Skill in applying and modifying the principles, methods, and techniques of professional nursing to provide on-going patient care. Skill in identifying problems and recommending solutions. Skill in preparing and maintaining records, writing reports, and responding to correspondence. Skill in developing and maintaining department quality assurance. Skill in establishing and maintaining effective working relationships with patients, medical staff, and the public. Ability to maintain quality control standards. Ability to react calmly and effectively in emergency situations. Ability to interpret, adapt and apply guidelines and procedures. Ability to communicate clearly.

Requirements

Education: Registered Nurse- OCN certification preferred

Experience: Two years of related oncology experience

Certificate/License: NYS nursing license required
