
The pharmacy medication reconciliation technician is a certified pharmacy technician who supports pharmacy services by obtaining medication and allergy histories for patients that will be admitted to the hospital. Information is obtained by interviewing the patient, patient family, calling pharmacies, long term carefacilitiesand/or other providers. The information obtained is communicated to pharmacists and health care providers todeterminetreatment plans and medication orders during hospitalization and post discharge from the hospital
Requirements
Registration with Indiana Board of Pharmacy within30 daysof hire date if you are a new registrant.
If previously registered, you must have registration upon hire.
