Job Description

Help Ensure Safe and Accurate Medication Care

We are seeking a detail-oriented and compassionate Medication History Specialist to join our Pharmacy team. In this role, you will work directly with patients and their families to gather accurate medication histories that help ensure safe and effective care throughout the hospital stay.

Your work plays a critical role in the medication reconciliation process, helping healthcare providers make informed decisions and reducing the risk of medication errors. You'll collaborate closely with pharmacists, nurses, providers, and other members of the care team in a supportive environment focused on quality, safety, and excellent patient care.

What You'll Do

  • Interview patients, family members, and caregivers to collect accurate prior-to-admission medication information
  • Document medication names, strengths, dosages, frequencies, and last known administration times
  • Review medication histories for completeness and accuracy
  • Communicate medication discrepancies and concerns to pharmacists for further evaluation
  • Support medication reconciliation efforts that promote patient safety and continuity of care
  • Maintain detailed and accurate documentation within the electronic medical record
  • Collaborate with pharmacists, providers, nurses, and other care team members
  • Protect patient privacy and maintain confidentiality in accordance with HIPAA guidelines
  • Follow established workflows and procedures to ensure consistent, high-quality documentation
  • Contribute to a culture focused on safety, teamwork, and exceptional patient care

What We're Looking For

  • High school diploma or equivalent
  • At least one year of experience in a healthcare, pharmacy, medical office, or clinical support setting
  • Strong attention to detail and documentation skills
  • Ability to communicate professionally and compassionately with patients and families
  • Comfortable working independently while managing multiple priorities
  • Ability to maintain confidentiality and handle sensitive information appropriately
  • Basic computer and electronic medical record skills
  • Knowledge of medication terminology or pharmacy workflows is helpful
  • A patient-focused mindset and commitment to accuracy
