Schedule: Monday–Friday | 8:00 a.m.–4:00 p.m.

Reporting to the Director, the MSW provides age-appropriate counseling, education, and support to patients and families in the home setting using standard social work practices. This role partners closely with patients, caregivers, physicians, and the interdisciplinary care team to develop and coordinate individualized plans of care.

Key Responsibilities

  • Assess patient and family needs and develop individualized plans of care
  • Provide counseling, behavioral health interventions, education, and community resource support
  • Coordinate care with physicians and interdisciplinary team members
  • Document patient progress, interventions, and outcomes in accordance with agency standards
  • Conduct hospice information visits for home care patients when requested
  • Support patient and caregiver education and discharge planning
  • Mentor and support new MSW staff and students

Requirements

  1. Master's degree in social work from an accredited program (Council on Social Work Education) OR bachelor's degree in social work with at least one year in healthcare care setting.
