Partnering with Elara Caring, a Medical Social Worker provides patient care across the entire continuum of care. The Medical Social Worker for our Service Line service line make a meaningful impact, caring for patients wherever it is they call home.

What is Required?

• Master’s degree or Doctoral degree in social work from a social work from a school of social work accredited by the Council of Social Work Education
• Current state license as a Social Worker
• 1 year of social work experience in a healthcare setting
• Social work skills as defined as generally social work standards
• 50% travel required
• Valid driver’s license and insurance and reliable transportation to perform job tasks

You will report to the Clinical Team Manager.
