As a Medical Records Clerk, you'll play a vital role in maintaining complete and accurate participant medical records while supporting our interdisciplinary care team. Responsibilities include:

  • Scan, upload, index, and accurately file medical records into the electronic health record (EHR).
  • Ensure documents are uploaded to the correct participant chart and labeled appropriately.
  • Review records for completeness, accuracy, signatures, dates, and required documentation.
  • Request and obtain medical records from hospitals, physician offices, specialists, and other healthcare providers.
  • Process release of information (ROI) requests in accordance with HIPAA and organizational policies.
  • Verify appropriate authorizations for the release of protected health information.
  • Process assigned order notifications and maintain applicable tracking logs.
  • Conduct routine chart audits and assist in correcting documentation discrepancies.
  • Help maintain a complete, organized, and compliant medical record for every participant.
  • Protect the confidentiality and security of participant information at all times.
  • Work collaboratively with nurses, providers, therapists, and other interdisciplinary team members.
  • Participate in team meetings, training, and other organizational initiatives.
  • Support participants by communicating requests for additional services to the appropriate care team member.

  • High school diploma or GED required.
  • Previous experience in medical records, health information management, medical office administration, or healthcare preferred.
  • Experience working with electronic medical records (EHR/EMR) strongly preferred.
  • Knowledge of HIPAA, medical terminology, and medical record documentation practices is preferred.
  • Proficiency with Microsoft Office and general computer applications.

Requirements

CERTIFICATION, LICENSES, REGISTRATIONS:

  • Certification in Health Information Management (HIM) preferred.

EDUCATION AND EXPERIENCE:

  • High School diploma or equivalent required.
  • A minimum of one-year experience working with the elderly population preferred.

KNOWLEDGE, SKILLS, and ABILITIES:

  • Must have basic computer, 10 key, and typing skills, knowledge of Excel, Microsoft Word, scanning and e-mailing, and a knowledge of office equipment operating and user maintenance procedures, including but not limited to phone system, photocopiers, fax machine, postage meter, etc.
  • Demonstrates a thorough understanding of HIPAA/HITECH laws and the related policies and procedures within the program. Attends and successfully completes mandatory HIPAA/HITECH training as required.
  • Knowledge of medical terminology required.
  • Knowledge of medical records retention requirements; knowledge of State and Federal requirements for health information management.
  • Must have the ability to give attention to small details, ensure completeness and accuracy of all work performed, and to be able to do so in a timely manner.
  • Ability to work under pressure to meet deadline, handle several tasks simultaneously.
  • Ability to follow directions and work independently with minimal supervision.
  • Must have excellent communication skills, be able to cope with mental and emotional stress related to the position, have flexibility and personal integrity.
  • Employee must demonstrate on an ongoing basis the ability to develop and maintain good working relationships with co-workers and department heads.
  • Works as a team player to meet common goals of the department.
  • Assists in the continuous quality improvement process.

PHYSICAL REQUIREMENTS: Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, occasional bending, reaching forward and overhead, squatting and kneeling. Ability to communicate verbally, with an excellent comprehension of the English language.
