ABOUT US

Founded in 2017, Upperline Health is the nation's largest multispecialty physician group dedicated to transforming value-based care (VBC). By breaking down traditional silos between specialists, primary care providers, payors, and patients, Upperline delivers a more connected and holistic care experience through our VBC program, Upperline Plus Our providers coordinate patients’ care and collaborate across a multidisciplinary team – including physicians, advanced practice providers, care navigators, pharmacists, dieticians, and social workers – to address both immediate and long-term patient needs.

Upperline serves patients nationwide, in clinic and at home, delivering expert care in chronic condition management, podiatry, vascular, endocrinology, wound care, primary care, and more.
