Position Medical Case Manager

Department Immigrant and Refugee Health Center

Location: Boston, MA

Schedule 40 hours per week, On-site

The Medical Case Manager is responsible for assessing the needs of patients, including comprehensive insurance, risk assessment, medical, adherence, and treatment needs; developing service plans based on those identified needs; providing outreach and facilitating referrals and access to social service benefits, concrete services, and other providers; monitoring patient progress and advocating on behalf of patients when they are unable to do so for themselves; complying with all data entry, data integrity, and data tracking requirements for funding agencies.

JOB RESPONSIBILITIES

  • Conducts Immigrant and Refugee Health Center (IRHC) Needs Assessment/Intake for new clients. Includes:
    • Mental health acuity/crisis assessment, and facilitate appropriate triaging
    • Comprehensive assessment of client’s medical, mental health and social needs
    • Collection of funder-required data points
    • Collaborate with front desk/administrative staff to schedule client for medical, mental health, social service appointments, as indicated by Needs Assessment
    • Appropriate documentation of needs assessment data in EMR
  • Serve as a key member of the IRHC Case Management Team
    • Respond to pages during regular business hours; meet clients in IRHC programs to assess acute and high-level biopsychosocial needs and ensure client is successfully connected with appropriate resources, services and providers.
    • Actively participate in ongoing development and refinement of case management model and processes in collaboration with IRHC teams
    • Provide consultation and recommendations to other hospital clinicians regarding resources and approaches to care for immigrant patients
  • Provide case management for established IRHC clients
    • Create a safe, warm, welcoming environment and encourage engagement in care
    • Provide case management services for established IRHC clients, including connecting patients to a broad range of services including but not limited to: mental health services, primary care services, medical specialty services, immigration legal services, financial assistance, food, transportation, housing, clothing, health literacy, interpreter service needs, etc.
    • Provide client education regarding available services and resources both within the IRHC, throughout BMC and in community, and provide tools and facilitate connections to these resources
    • Assist in facilitation of case management groups for IRHC patients
    • Facilitates interdisciplinary consultation on patient’s behalf through participation in rounds, team meetings and clinical reviews
    • Maintains HIPAA standards and confidentiality of protected health information.
    • Performs and maintains all documentation in the electronic chart of patient encounters; resource development efforts; and other patient interventions and interactions
  • Program development and administrative responsibilities include:
    • Participates in multidisciplinary/staff/team meetings and activities; meets regularly with leaders at IRHC and specialty programs to triage program issues appropriately.
    • Participates in team-based Quality Improvement (QI) initiatives
    • Participates in local site operations, including team operational meetings; and is actively engaged in developing, implementing and refining operational protocols and processes as IRHC infrastructure expands
    • Encouraged to engage in learning and growth opportunities including attending weekly Grand Rounds, and pursuing training opportunities with the approval of supervisor

JOB REQUIREMENTS

EDUCATION:

  • Bachelor’s degree required. Master’s degree (or equivalent) in social work, psychology, or related human services field preferred.

  • However, an equivalent combination of education and experience or specific certifications/licensures, which provides proficiency in the areas of responsibility, may be substituted for the education and experience requirements

EXPERIENCE:

  • 2-3 years of experience working in a medical or social services environment.

CERTIFICATES, LICENSES, REGISTRATIONS

  • Adult Mental Health First Aid, LCSW, LICSW or LMHC as appropriate, preferred.

KNOWLEDGE AND SKILLS:

  • Bilingual-bicultural preferred.

  • Able to employ clinical skills to create service plan interventions based on an objective assessment tool.

  • Expertise in establishing working relationships with patients, their families, professionals, and the community.

  • Knowledge of community, social, and health resources

  • Facility performing multicultural assessments and interventions.

  • Sensitivity to ethnic, cultural, gender, and sexual orientation diversity, values, beliefs, and behaviors.

  • Demonstrated ability to be flexible with client’s priorities, evolving needs, and goals.

  • Demonstrated empathic understanding of patients who may have, mental illness, substance abuse or other health issues.

  • Effective verbal and written communication skills in English.

  • Demonstrated ability to write contract reports that are clear and informative. Demonstrated critical thinking and problem solving ability.

  • Computer competency, including Microsoft Outlook, Work, and Excel.

  • Effective interpersonal skills to facilitate communication with various members of the health care team, patients, and their families.

Compensation Range:

$53,000.00- $77,000.00

This range offers an estimate based on the minimum job qualifications. However, our approach to determining base pay is comprehensive, and a broad range of factors is considered when making an offer. This includes education, experience, skills, and certifications/licensures as they directly relate to position requirements; as well as business/organizational needs, internal equity, and market-competitiveness.
