Provides outpatient HIV medical case management services and patient advocacy through education, counseling, advocacy, benefits assessment and enrollment, and coordination of services. Ensures patient access to essential medications, medical care, and support services. Provides services to people living with HIV/AIDS who are medically managed through the grant-funded Ryan White Program – Division of Infectious Diseases.

Essential Functions

  • Performs medical case management functions for patients/clients of the Ryan White Program. Provides treatment adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments.

  • Responsible for intake and assessment of service needs.

  • Develops a comprehensive, individualized service/action plan.

  • Coordinates services required to implement the plan.

  • Responsible for client monitoring to assess the efficacy of the plan.

  • Responsible for benefit assessments and enrollment, e.g. Medicaid, Medicare, ACA, and ADAP.

  • Provides coverage in the duty room to assist clients with Patient Assistance program applications.

  • Serves as a member of the interdisciplinary health team.

  • Performs other duties as assigned.

Supervisory/Management Responsibilities

  • This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

  • Education - Bachelor’s degree from an accredited School of Social Work or related field of study. Licensed Bachelor of Social Work (LBSW) preferred.

  • Experience - One (1) year experience in health or social services related area.
