
New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast (and beyond), serving 15,000 children and adults annually through inpatient crisis services, 24-hour help line and mobile response team, and outpatient programs conveniently located across Indian River, Martin, Okeechobee and St. Lucie counties. New Horizons is looking for an Medical Care Coordinator who will provide comprehensive nursing-based care coordination services for individuals with behavioral health conditions, substance use disorders, co-occurring disorders, and complex medical needs. This role is responsible for coordinating and monitoring medical and psychiatric care to promote continuity of services, improve health outcomes, and reduce avoidable emergency room visits and hospitalizations. Services are delivered utilizing a person-centered, recovery-oriented, and trauma-informed approach in accordance with Florida DCF Care Coordination guidelines.
As a Medical Care Coordinator , your responsibilities will include:
· Serve as the primary nursing care coordinator for assigned individuals.
· Conduct timely follow-up and care coordination related to high utilizers, emergency department visits, hospitalizations, and medical send-outs.
· Identify medically complex and high-risk individuals requiring enhanced care coordination and intervention.
· Coordinate primary care, specialty care, psychiatric, pharmacy, and behavioral health appointments.
· Assist with discharge planning and continuity of care following inpatient detox admissions.
· Monitor medication continuity, treatment compliance, and follow-up recommendations.
· Coordinate care with hospitals, healthcare providers, pharmacies, managed care organizations, and community agencies.
· Assist individuals with accessing insurance benefits, medication assistance programs, transportation services, and community-based resources.
