🩺MDS RN – Full Time | Added to On-Call Rotation After 90 Days
Location: White Oak Manor York - 111 S Congress Street, York, SC 29745
We’re hiring a RN Nurse Assessment Coordinator (NAC)RN Nurse Assessment Coordinator (NAC) perform precise, timely resident assessments, uphold regulatory standards, and drive high-quality, person-centered care. After 90 days, this role will be added to the on-call rotation. Wage Scale Based on YOUR years of experience.
What You'll Do:
♦ Conduct and coordinate accurate resident assessments in accordance with regulatory requirements
♦ Develop, update, and monitor individualized care plans based on assessment findings
♦ Collaborate with interdisciplinary teams to ensure quality, person-centered care
♦ Train, mentor, and provide guidance to nursing staff on assessment processes
♦ Ensure compliance with state and federal regulations, documentation standards, and facility policies
♦ Review data to identify trends, opportunities for improvement, and quality initiatives
♦ Serve as a resource for staff and residents regarding assessment procedures and care planning
Qualifications:
♦ Current RN license South Carolina
♦ Long-term care or MDS experience preferred
♦ Strong assessment, documentation, and teamwork skills
