Reports To: Director of Nursing, Administrator

Position Summary

The MDS RN is responsible for completing the Minimum Data Set (MDS) for each resident in accordance with all applicable state and federal regulations to ensure an accurate and comprehensive assessment of the resident's clinical status and care needs.

Essential Duties and Responsibilities

• Evaluate the overall status of residents through observation, medical record review, and interviews with residents and interdisciplinary team members to ensure the completion of an accurate Minimum Data Set (MDS).
• Complete the MDS in a timely manner in accordance with state and federal regulations and established facility policies.
• Enter MDS assessments into the computer system and transmit assessment data in accordance with state and federal requirements.
• Print Care Area Trigger (CAT) and Care Area Assessment (CAA) reports and complete or assist with their completion.
• Upon completion of the CATs and CAAs, collaborate with the interdisciplinary team to develop and complete individualized resident care plans.
• Enter resident care plans into the computer system.
• Update or assist with updating resident care plans as needed.
• Communicate with residents, family members, physicians, interdisciplinary team members, and other healthcare professionals regarding resident status and care planning needs.
• Be capable of providing nursing care on the floor when needed.
• Demonstrate knowledge of and promote resident rights.
• Recognize, prevent, and report suspected abuse, neglect, and exploitation of residents in accordance with facility policy.
• Promote and maintain a safe, clean environment in which residents may live.
• Review the Twenty-Four Hour Report daily to remain aware of changes in residents' conditions and/or plans of treatment and determine when a Significant Change MDS is required.
• Perform all other duties and responsibilities as assigned by the Director of Nursing or RN Coordinator.
• Serve as an active member of the facility team by placing residents as the highest priority.
• Maintain communication with the Nursing, Social Services, Activities, Dietary, and Therapy departments regarding the MDS schedule.

Talking: Communicating with residents, families, physicians, visitors and staff

Hearing: Taking instructions from the Administrator, To respond to resident complaints and requests, To respond appropriately to disaster instructions and pages

Sight: For performing job effectively and correctly

Smell: For accurate detection and maintenance of facility odors

Touch: In order to detect changes in temperature

LICENSURE REQUIREMENTS:

Must be an R.N. currently licensed and in good standing with the Board of Nursing in the state of Alabama

SUMMARY OF OCCUPATIONAL EXPOSURE:

Tasks and procedures performed by the employee involve risks classified by C.D.C. as Category I. (Direct contact with blood or other bodily fluids to which universal precautions apply.)

OTHER CONSIDERATIONS AND REQUIREMENTS:

Must be able to tolerate a high pace as typical for a nursing facility. Must be able to take and give instructions well and enjoy working with the elderly. Although employee is required to sit for prolonged periods of time, he/she must also be able to tolerate prolonged standing and walking.
