Marin Post Acute is seeking an experienced and detail-oriented MDS Coordinator (RN or LVN) to join our dedicated clinical leadership team. This vital role is responsible for overseeing the MDS process, ensuring regulatory compliance, optimizing reimbursement, and supporting exceptional resident outcomes.

What You'll Do

  • Coordinate and manage the MDS/RAI process in accordance with federal, state, and CMS regulations
  • Complete accurate and timely MDS assessments, care plans, and supporting documentation
  • Collaborate with the interdisciplinary team (IDT) to develop individualized, resident-centered care plans
  • Monitor and improve quality measures, compliance standards, and reimbursement accuracy
  • Provide education and guidance to nursing staff regarding MDS processes and regulatory updates
  • Assist with audits, reporting, quality improvement initiatives, and survey readiness activities
  • Partner with facility leadership to support clinical excellence and operational success

Qualifications

  • Current and active California RN or LVN license
  • 1+ year of MDS/RAI experience in a skilled nursing or post-acute setting preferred
  • Strong understanding of PDPM, CMS reimbursement systems, care planning, and regulatory compliance
  • Excellent organizational, analytical, and documentation skills
  • Ability to work collaboratively with clinical, administrative, and leadership teams
  • Commitment to delivering high-quality resident care and positive outcomes
