LPN MDS Nurse
We are seeking a detail oriented and knowledgeable Licensed Practical Nurse (LPN) MDS Nurse to join our long term care team. The MDS Nurse is responsible for coordinating and completing the Minimum Data Set (MDS) process in accordance with federal and state regulations. This role ensures accurate assessment, documentation, and care planning to support both quality resident outcomes and proper reimbursement.

Key Responsibilities
• Complete accurate and timely MDS assessments and related documentation.
• Develop and update individualized care plans based on resident assessments.
• Coordinate interdisciplinary team (IDT) involvement in the care planning and MDS process.
• Monitor regulatory compliance related to RAI processes and resident documentation.
• Ensure accuracy of Medicare, Medicaid, and insurance documentation to support appropriate reimbursement.
• Review resident charts for changes in condition, medication updates, and treatment needs.
• Participate in quality improvement initiatives and audits.
• Communicate effectively with nursing staff, therapy, social services, dietary, and administrative teams.
• Provide education and support for staff regarding assessment and documentation standards.
• Maintain strong working knowledge of RAI, PDPM, and state specific requirements.
Qualifications
Required:
• Active Ohio LPN license in good standing.
• Experience in long term care or skilled nursing facility (SNF).
• Working knowledge of MDS 3.0, care planning, and regulatory guidelines.
• Strong attention to detail and documentation accuracy.
• Excellent communication, organizational, and teamwork skills.
• Proficiency with electronic medical records (EMR) systems.
Preferred:
• Prior MDS/Resident Assessment Coordinator experience.
• Familiarity with PDPM and reimbursement practices.
• RAC CT certification (or willingness to obtain).
