Diablo Valley Post Acute is seeking an experienced and detail-oriented MDS Nurse (LVN/LPN) to join our team. The MDS Nurse plays a critical role in coordinating and completing accurate Minimum Data Set (MDS) assessments while ensuring compliance with federal and state regulations.

This position works closely with the interdisciplinary team to support individualized care planning, accurate clinical documentation, regulatory compliance, and appropriate Medicare/Medicaid reimbursement.

Key Responsibilities

As an MDS Nurse, you will be responsible for ensuring accurate, timely, and compliant MDS assessments and documentation.

Responsibilities include:

  • Complete MDS assessments and Care Area Assessments (CAAs) within required timelines.

  • Ensure accurate clinical documentation to support resident care planning and reimbursement.

  • Maintain compliance with CMS, state, and facility regulations and guidelines.

  • Collaborate with nursing, therapy, social services, dietary, and other interdisciplinary team members.

  • Develop and update individualized resident care plans based on assessment findings.

  • Monitor resident progress and revise care plans as needed.

  • Participate in resident care conferences and quality improvement initiatives.

  • Ensure MDS submissions meet regulatory requirements for Medicare and Medicaid reimbursement.

  • Assist with regulatory audits and surveys by providing accurate documentation and reports.

  • Stay current with changes to MDS, RAI, CMS, and skilled nursing regulations.

  • Provide education and guidance to staff regarding documentation standards and MDS requirements.

  • Communicate effectively with residents and families regarding care planning and resident needs.

  • Maintain accurate records within the facility's electronic health record (EHR) system.

Qualifications

  • Current LVN/LPN license in the state of practice.

  • Previous experience in long-term care or a skilled nursing facility preferred.

  • Knowledge of MDS 3.0, RAI processes, and Medicare/Medicaid reimbursement guidelines

  • Strong organizational skills and attention to detail.

  • Excellent written and verbal communication skills.

  • Ability to work effectively with an interdisciplinary team.

  • Proficiency with electronic health records (EHR) systems and Microsoft Office.

  • Ability to maintain required continuing education and professional licensing.

  • Must remain in good standing with the State Board of Nursing.
