Now Hiring: Part-Time MDS Nurse (LVN)

San Bruno Skilled Nursing is seeking a knowledgeable and detail-oriented MDS Nurse (LVN) to join our team on a part-time basis.

  • Schedule: Part-Time
  • Pay Rate: $50k-60k (DOE)

Key Responsibilities

  • Coordinate and complete MDS assessments, Care Area Assessments (CAAs), and related documentation within required timelines.
  • Ensure clinical documentation accurately reflects resident conditions and supports reimbursement processes.
  • Maintain compliance with CMS, state, and facility regulations.
  • Collaborate with the interdisciplinary team to develop and update individualized care plans.
  • Monitor resident progress and ensure care plan revisions are completed as needed.
  • Participate in care conferences and quality assurance activities.
  • Ensure timely and accurate MDS submissions for Medicare and Medicaid reimbursement.
  • Assist during audits, surveys, and regulatory reviews.
  • Provide education and guidance to staff regarding documentation standards and MDS requirements.
  • Communicate effectively with residents, families, and healthcare team members regarding care planning and resident needs.
  • Stay informed of regulatory updates affecting MDS processes and skilled nursing documentation.

Qualifications

  • Current California LVN license in good standing.
  • Previous experience in skilled nursing, post-acute care, or long-term care preferred.
  • Working knowledge of MDS 3.0, the RAI process, and Medicare/Medicaid reimbursement guidelines.
  • Strong organizational, analytical, and documentation skills.
  • Ability to manage multiple deadlines while maintaining accuracy and compliance.
  • Experience with electronic health record (EHR) systems.
  • Proficiency in Microsoft Office applications.
  • Strong communication and collaboration skills.
