Key Responsibilities For MDS Coordinator:

  • Lead and oversee clinical care programs in compliance with organizational standards, SC Nurse Practice Act, and federal certification guidelines.
  • Demonstrate knowledge of the survey process, educate and assist the staff in becoming aware and comfortable with the process.
  • Coordinate and oversee individualized care plans, ensuring nursing care aligns with resident wishes, advance directives, and all regulatory requirements.
  • Drive quality improvement through planning, implementing, and evaluating QA initiatives.
  • Support admissions by assessing resident needs to ensure appropriate community care placement
  • Coordinate the interdisciplinary team in implementing the RAI process in accordance with current federal and state rules, regulations and guidelines.
  • Ensure accurate completion of assessments and care plans through interdisciplinary review and communication with staff, physicians, and families
  • Support Medicare coverage decisions, set ARDs, and ensure compliance with Medicare requirements
  • Disseminate any new or updated materials involving the RAI process to the interdisciplinary team.
  • Ensure timely electronic submission and general validity of all MDS’s to the federal and state database.

Requirements:

  • Licensed Practical Nurse or Registered Nurse in the State of South Carolina
  • Two years of clinical experience in the healthcare setting
  • One year of experience in a long-term care setting
  • One year of experience as MDS Coordinator
  • Strong clinical assessment skills
  • Intermediate computer skills
  • CPR certified
  • Valid SC Driver’s License
