At Pine Ridge Post Acute, we believe quality care starts with a compassionate, dedicated team. We’re looking for an organized and detail-oriented MDS Coordinator to join our team and help ensure our residents receive individualized, high-quality care.

What You’ll Do

  • Coordinate and complete Minimum Data Set (MDS) assessments accurately and within required timelines, in accordance with federal and state regulations.
  • Develop, review, and update individualized resident care plans in collaboration with physicians, nursing staff, therapy, social services, residents, and families.
  • Facilitate resident care conferences and promote effective communication among the interdisciplinary care team.
  • Ensure resident records and clinical documentation accurately reflect the care provided and support appropriate reimbursement.
  • Monitor residents’ conditions and coordinate follow-up to ensure their individual care needs are met.
  • Assist with admissions, transfers, discharges, and timely completion of required assessments and documentation.
  • Support regulatory compliance, quality assurance initiatives, and facility policies and procedures.
  • Participate in leadership meetings, quality improvement activities, and other clinical initiatives as needed.
  • Promote resident rights, dignity, safety, confidentiality, and a positive care environment.
  • Collaborate with the nursing team to identify opportunities to improve care delivery and resident outcomes.

What We’re Looking For

  • Current, active, and unencumbered RN or LPN license in the state of practice.
  • Nursing degree from an accredited program.
  • At least one year of experience in geriatric nursing, including medication administration and related clinical responsibilities.
  • Previous MDS experience strongly preferred.
  • Strong organizational skills and exceptional attention to detail.
  • Excellent communication, documentation, and problem-solving abilities.
  • Ability to work collaboratively with residents, families, and an interdisciplinary team.
  • Flexibility, professionalism, and a genuine commitment to resident-centered care.

RN licensure is strongly preferred. LPN candidates may be considered with appropriate RN oversight.
