We are seeking an experienced and highly organized Registered Nurse (RN) MDS Coordinator to oversee Medicare assessments, care planning, and reimbursement-related activities within our Long-Term Care facility. This is an excellent opportunity for an RN who understands the MDS process, regulatory requirements, and the importance of accurate clinical documentation.
Under the direction of the Director of Nursing – Long-Term Care, the MDS Coordinator plays a key role in ensuring that resident assessments and care plans are accurate, timely, clinically appropriate, and compliant with New York State Department of Health regulations and applicable reimbursement requirements.
The MDS Coordinator is an essential member of the Long-Term Care team. Your work helps ensure that residents receive individualized, high-quality care while supporting accurate documentation, regulatory compliance, and appropriate reimbursement.

The MDS Coordinator must demonstrate the knowledge and skills necessary to support the care of adult and geriatric residents, including an understanding of age-specific needs, growth and development across the lifespan, and the ability to assess and interpret clinical information to support individualized care.
Occupational exposure to bloodborne pathogens is reasonably anticipated in this position.
