Job Description

Westminster Place is seeking a dedicated Part Time MDS Coordinator for the Nursing Administration department. Join the team at Westminster Place, a Presbyterian Living community, an independent, non-profit senior living care organization offering full continuum of care from independent and assisted living to skilled nursing and memory care at four luxury retirement communities.

Westminster Place - MDS Coordinator - Part Time - (9am-5pm) - Skilled Nursing Department.

Compensation: $41.84 per hour - $62.71 per hour (Salary based on years of experience).

Job Responsibilities:

  • Ensures the completion of the Resident Assessment Instrument (MDS) within federally mandated guidelines and timelines for each resident.
  • Leads the RAI team in guidance for PDPM requirements. Ensures that correct diagnosis for maximum reimbursement is chosen based on PDPM guidelines.
  • Processes CATs and CAAs for admission, 14-day, annual, and significant change MDS, evaluates and adds to resident Care Plan as appropriate for both Medicare Part A and non-Medicare residents.
  • Determines, at readmission of a resident, whether a new MDS must be completed or the previous one is adequate, depending upon whether or not there has been a major, permanent change in the resident's condition.
  • Notifies all involved disciplines of the following:
    • Date an admission MDS is due.
    • Whether a new MDS must be initiated for a resident who has been readmitted and date it is due.
    • Date a new MDS must be completed for a significant change in condition.
    • Date an annual MDS is due.
    • Date a quarterly MDS is due.
    • Date a Medicare required MDS (e.g. 5-day, 14-day, etc.) is due.
    • Date an OMRA (other Medicare required assessment) is due.
  • Ensures that a minimum of 15 months MDS information is available for each resident in a skilled bed.
  • Maintains personal records to track admission, Medicare, quarterly, annual, readmission, and significant change reassessment MDS for each resident.
  • Assists and instructs nursing personnel as well as other disciplines in the correct process for completing the MDS.
  • Transmits MDS information to the Federal Government, CMS, for residents in all certified beds in a timely fashion. Maintains transmission log and validation reports.

Education: Graduation from an approved registered professional nursing program.

Qualifications:

  • Active, current Illinois license to practice professional nursing.
  • CPR Certification Required - Accredited by American Heart Association.
  • Wound care certification or willingness to complete certification within 1 year of starting the job.
  • One year as an RN in a hospital (general care) or long-term care setting with adults preferred.
  • Experience with MDS and Care Plans recommended.
  • Knowledge of Medicare regulations and MDS 3.0.
  • AANAC Certification a plus.
