Job Title: MDS Coordinator

$2,500 Sign On Bonus

Requirements:

• Graduate of an accredited school of nursing.
• LPN/LVN or RN license required.
• Working knowledge of all regulatory and compliance standards related to MDS.

Responsibilities:

• Coordinates and completes comprehensive resident assessments (MDS) and ensures accuracy and timeliness of assessment data.
• Serves as a resource person for resident care planning activities and conducts orientation sessions on the MDS process.
• Collaborates with the interdisciplinary team on care planning and supports regulatory compliance and reimbursement accuracy.
