MDS Coordinator (LPN or RN)

We are seeking a strong, detail-oriented MDS Coordinator to oversee MDS assessments, care planning, and reimbursement processes in compliance with Medicare, Medicaid, and state regulations. This role is critical to ensuring clinical accuracy, regulatory compliance, and optimal resident outcomes.

Key Responsibilities:

• Complete and manage accurate, timely MDS assessments in accordance with federal and state guidelines
• Collaborate with the interdisciplinary team to develop and update individualized care plans
• Ensure compliance with Medicare/Medicaid regulations and documentation standards
• Utilize EHR systems to maintain precise clinical documentation
• Support reimbursement optimization and clinical documentation improvement
