Salary: $100,000 to $105,000
Schedule: Full Time, 9:00am - 5:00pm, Monday- Friday

What We Offer:
Blakehurst is looking for an experienced, hospitality focused MDS Coordinator that brings compassion, professionalism, and strong skills to work every day. The MDS Coordinator Nurse is an RN who is responsible for the overall supervision, coordination, maintenance and timely completion of the MDS, care plans and routine assessments for residents.
Primary Responsibilities:
Essential Job Duties
1. Coordinates, conducts, and completes the MDS on the computerized system,
and completes resident care plans. Completes Service Plans for Assisted Living
residents.
2. Assures accuracy and completeness of the medical record.
3. Assures and monitors that MDS’s are completed within state and federal
guidelines.
4. Ensures MDS electronic submission occurs per State regulations.
5. Coordinates and participates in family/resident care plan conferences and
service plan conferences.
6. Assures effective communication with other shifts and departments as it relates
to medical record documentation.
7. Communicates RUGS levels with accounting department to ensure accurate
billing and reimbursement.
8. Responsible for preparing and submitting OCAP and Benchmark reports.
9. Monitor Relias training to ensure compliance for all Chestnut Green staff.
10. Conduct needed education sessions and ensure that all applicable staff
complete the training.
Other Duties:
1. Serves as a staff nurse when staffing levels necessitate in order to meet the
needs of the residents.
2. Responds to emergency calls as necessary.
3. Assures timeliness of physician’s visits.
4. Notifies physicians and family of changes in resident’s condition.
5. Encourages rehabilitative techniques to restore residents’ independence.
6. Assists DON in developing and implementing medical records policies and
procedures.
7. Attends in-service training and education sessions, as assigned.
8. Performs specific work duties and responsibilities assigned by supervisor.
9. Participate in morning meeting each day and ensure that the care plan is kept up
to date at all times.
10. Participate in the monthly Quality Assurance meeting by preparing and
presenting the facility Quality Indicator and Quality Measure Reports.
11. Participate in the nurse manager weekend on call program.
QUALIFICATIONS
RN currently licensed by State of Maryland. Current CPR certification. In- depth experience and knowledge working with MDS computer software. Experience with the MDS transmittal process and understanding of regulations impacting the MDS. Supervisory experience preferred.
