Creates and executes targeted marketing strategies, cultivating referral relationships, and serving as a key representative to external partners for Nathaniel Witherell in order to promote census development, while supporting the admissions process in order to ensure timely, appropriate, and smooth transitions for new admissions.

Examples of duties

Education and Experience:
Bachelor’s degree from an accredited institution in Nursing, Social Work, or a related field, plus three (3) years of healthcare admissions and census development experience, preferably in a skilled nursing or post-acute care setting.

Qualifications:
Demonstrated knowledge of long-term care admissions process, including regulatory guidelines, and payer systems.

Proven marketing and communication skills, within a healthcare environment.

Proven ability to build rapport with diverse referral sources and families.

Demonstrated organizational skill with strong follow-through, time management, and problem-solving abilities.

Proven ability to work independently and prioritize tasks in a fast-paced, target-driven environment.

Demonstrated proficiency in Microsoft Office applications and relevant Customer Relationship Management (CRM) software or admissions tracking systems.

Demonstrated ability to deal tactfully and courteously with colleagues, residents, families, and healthcare professionals.

Examination: 100% Training & Experience Assessment

The examination to determine a candidate's eligibility for the position will be a Training and Experience Assessment. Duties and Responsibilities:

Leads proactive outreach efforts to hospitals, assisted living facilities, physician offices, home health agencies, and other potential referral sources to drive admissions and increase census.

Maintains ongoing contact with hospital discharge planners, social workers, and other referral coordinators to identify and convert high-potential referrals.

Builds and nurtures strong professional relationships with key stakeholders in the community to promote the facility’s services and generate consistent referral streams.

Attends community events, professional association meetings, and networking functions to represent the facility and support brand visibility.

Tracks and analyzes referral trends to inform outreach strategies and monitor census development performance.

Provides facility tours, responds to inquiries, and educates prospective residents and families on available services and insurance/payment options.

Collaborates with hospital case managers, physicians, and families to ensure appropriate placement and timely admissions

Oversees end-to-end admissions processes, including coordinating with nursing, medical, therapy, and finance departments for efficient transitions.

Oversees insurance verifications, including pre-certification and authorization procedures, ensuring alignment with payer requirements and internal policies.

Ensures accurate documentation of all inquiries and applications in accordance with regulatory and organizational standards.

Maintains and regularly updates waitlists, inquiry logs, and marketing databases.

Follows up with newly admitted residents and families to ensure satisfaction and to identify additional referral opportunities.

Works closely with the Director of Admissions to align admissions efforts with census targets and strategic goals.

Partners with interdisciplinary team members to resolve medical or financial issues impacting admission eligibility.

Prepares and distributes admission packets, documents, and required records to appropriate departments.

Ensures adherence to HIPAA Privacy Regulations when handling resident information.

Maintains current knowledge of Medicare, Medicaid, and commercial insurance requirements.

Monitors marketing materials and promotional supply levels to support outreach efforts.

Supports Town policies and philosophies.

Performs related work assigned.
