Program Overview – Supportive Housing Programs, Community Care Unit
(Cummer Housing: 185 Cummer Avenue, Toronto, ON)
This position is responsible to the Director, Supportive Housing Programs, Community Care.
The Manager, Supportive Housing Programs oversees the day-to-day operations site, including supervision of supervisors, tenants, program staff, volunteers and students. This position is responsible for the daily implementation of services and supporting the team with achieving the program outcomes. The Manager is responsible for implementing protocols, policies and procedures, and program development.
The Supportive Housing Program provides housing for seniors who have been street involved, experiencing mental health and/or addictions issues. The program provides onsite psychosocial, primary care supports and services including: intensive case management; harm reduction and addiction supports; therapeutic and supportive counseling; assistance with activities of daily living; nursing services; life skills development; social and recreational activities; volunteer opportunities; and follow up support.
