RESPONSIBILITIES:
The System Manager of Social Work will oversee and coordinate social work services across INTEGRIS Health, ensuring comprehensive, patient-centered care that aligns with the organization’s mission, vision, and values. This leadership role will be responsible for developing, implementing, and evaluating social work programs across the organization. The System Manager will provide supervision, mentorship, and professional development for social work staff while ensuring adherence to the highest standards of clinical practice and ethical guidelines. The System Manager will work closely with other health care teams to address the psychosocial needs of patients and their families, aligning all services with INTEGRIS Health’s strategic initiatives, key performance indicators (KPIs), and key process standard work. The role also focuses on optimizing care transitions, reducing length of stay (LOS), improving patient outcomes, and enhancing collaboration with community resources to support patients’ post-discharge needs. This position is crucial in enhancing patient satisfaction, improving care coordination, and ensuring compliance with federal, state, and institutional regulations and policies.
Leadership and Management:
- Provide strategic leadership for social work services across the hospital system, ensuring alignment with INTEGRIS Health’s mission, vision, and values.
- Ensure that all social work practices and initiatives support and reinforce INTEGRIS Health’s strategic priorities and organizational goals.
- Directly supervise social work supervisors and staff, providing regular feedback, performance evaluations, and professional development opportunities while holding the team accountable to the adherence to INTEGRIS Health’s values.
- Develop and maintain positive working relationships with key stakeholders, including physicians, nursing staff, department heads, and leadership teams, to foster a collaborative environment.
- Promote an organizational culture that emphasizes teamwork, respect, accountability, and a commitment to patient care in alignment with INTEGRIS Health’s values.
Program Development and Implementation:
- Oversee the design and execution of evidence-based social work programs aimed at improving patient outcomes, reducing hospital readmissions, and enhancing the patient experience, in alignment with INTEGRIS Health’s strategic initiatives.
- Collaborate with interdisciplinary teams to ensure social work services are effectively integrated into the patient care model and aligned with the key process standard work of INTEGRIS Health.
- Develop programs to improve care transitions, ensuring patients are effectively transitioned from the hospital to home or other care settings with appropriate follow-up services.
- Work to reduce length of stay (LOS) by ensuring timely and efficient discharge planning, addressing psychosocial factors, and utilizing community resources to support post-discharge care.
- Ensure that social work services are consistently evaluated and adjusted to meet the evolving needs of the organization and the diverse patient populations we serve.
Clinical Oversight and Patient Care:
- Provide clinical oversight to ensure social workers deliver high-quality, patient-centered services across inpatient, outpatient, and specialty care settings.
- Serve as a resource for complex cases requiring advanced clinical interventions, including crisis management, trauma-informed care, and high-risk assessments.
- Oversee the development and implementation of psychosocial assessments, care plans, and discharge planning to meet patient and family needs, ensuring alignment with INTEGRIS Health’s mission and strategic objectives.
- Utilize community resources to facilitate patients’ post-discharge needs, including referrals to local services, home health agencies, mental health providers, and financial assistance programs.
- Ensure that social work services adhere to ethical standards, regulatory requirements, and industry best practices.
Quality Improvement and Compliance:
- Lead quality improvement initiatives that enhance the effectiveness and efficiency of social work services across the system, ensuring alignment with INTEGRIS Health’s KPIs.
- Monitor and report on key performance indicators (KPIs), patient satisfaction data, and regulatory compliance related to social work practices, ensuring all efforts contribute to INTEGRIS Health’s strategic goals.
- Ensure the department adheres to all relevant legal, regulatory, and accreditation requirements (e.g., CMS, Joint Commission, state and federal laws), reinforcing INTEGRIS Health’s commitment to excellence.
- Respond to and resolve any issues related to social work services, including complaints, grievances, or compliance concerns, in alignment with the organization’s goals.
Training, Education, and Professional Development:
- Provide ongoing supervision and mentorship to social work staff, including guidance on complex cases, ethical dilemmas, and advanced clinical interventions, ensuring they adhere to organizational values.
- Ensure staff are up to date on new clinical practices, regulatory changes, and advances in social work, and that educational efforts contribute to the achievement of organizational and departmental goals.
Budgeting and Resource Management:
- Assist with budgeting and financial planning for the social work department, ensuring resources are allocated effectively and efficiently to meet both clinical and operational objectives.
- Monitor expenditures and manage the social work department’s budget in collaboration with senior leadership to ensure financial responsibility and sustainability, aligned with INTEGRIS Health’s fiscal goals.
Community Resources and Post-Discharge Support:
- Identify and establish partnerships with community resources and organizations to facilitate patients’ transition to post-discharge care, including mental health services, rehabilitation, home care, financial assistance programs, and other support services.
- Collaborate with community-based organizations to ensure smooth transitions from the hospital to home or other care settings, improving care continuity and reducing avoidable readmissions.
- Ensure effective communication with community organizations and services to promote patient access to necessary post-discharge care and support, in alignment with INTEGRIS Health’s goals to provide holistic, patient-centered care.
Community Relations and Advocacy:
- Represent the social work department in meetings with external stakeholders, including government agencies, community organizations, and advocacy groups, ensuring that INTEGRIS Health’s values and strategic priorities are effectively communicated.
- Advocate for policies that improve patient access to services and ensure equitable health care delivery.
- Ensure that social workers are involved in community outreach, education, and advocacy initiatives as needed, supporting the hospital system’s commitment to community health and wellness.
REQUIRED QUALIFICATIONS:
EXPERIENCE:
- Minimum of 5 years of experience in clinical social work, with at least 2 years in a leadership or management role within a health care setting.
- Strong understanding of health care regulations, patient privacy laws (HIPAA), and accreditation standards (e.g., The Joint Commission, CMS).
- Demonstrated ability to lead, mentor, and manage social work staff in alignment with organizational values and strategic goals.
EDUCATION:
- Master of Social Work (MSW) from an accredited program.
LICENSE/CERTIFICATIONS:
- Licensed Clinical Social Worker (LCSW) or equivalent, with eligibility for licensure in the state of Oklahoma.
SKILLS:
- Exceptional leadership, organizational, and interpersonal skills.
- Strong clinical judgment and ability to make decisions in complex, high-pressure situations.
- Excellent verbal and written communication skills, with the ability to interact with diverse populations and work as part of a multidisciplinary team.
- In-depth knowledge of social work practice, including individual, family, and group therapy, crisis intervention, and case management.
- Experience with community resources and post-discharge support services.
- Experience with electronic medical records (EMR) systems and other health care technology.
- Ability to manage multiple projects, set priorities, and meet deadlines.
- Knowledge of quality improvement processes and outcomes-based care models, with a focus on reducing length of stay (LOS) and improving care transitions.
This job requires the incumbents to operate an INTEGRIS-owned vehicle OR personal vehicle (non-INTEGRIS-owned) and therefore must have a current Oklahoma State Driver's License as well as a driving record which is acceptable to our insurance carrier.
PREFERRED QUALIFICATIONS
EXPERIENCE
- Experience in a hospital or large health care system preferred.
