We are hiring a Manager of Quality, Patient Safety and Risk!

Closing Date: June 3, 2026

Salary: The salary range for this role is $90,434- $109,591per annum. *This represents steps 1-7 of a seven-step salary grid. Candidate experience and internal equity will be considered when determining the starting salary

Location: The home office is located at 1101 Nicholson Rd, Unit 5, Newmarket, Ontario. Due to the nature of this role, candidates will be required to attend events in and around York Region and South Simcoe with regular travel in York Region.

Job Profile

As a leadership team member, the Manager of Quality, Patient Safety, and Risk provide the expertise, tools, and means to promote continuous quality improvement, a culture of safety, and risk awareness at CMHA. This involves designing and managing processes and activities throughout the organization on an ongoing basis to achieve excellence. The Manager will lead the accreditation process, provide coaching to CMHA staff in the areas of risk management, patient safety, and continuous quality improvement, engage clients/families, staff, and stakeholders in continuous quality improvement, and lead organizational-level QI projects. This role is directly supervised by the Director, of Operational Excellence.

Duties Include but are not limited to:

  • Managing the full accreditation cycle and acts as the CMHA-YRSS Accreditation Coordinator
  • Leads the interpretation of accreditation tool results, and supports other leaders in the development of action plans
  • Identifies areas of potential non-compliance and assists in developing remedial action to meet compliance
  • Develop strategies for ongoing compliance, embedding accreditation criteria into daily work so that CMHA YRSS remains in a state of ongoing accreditation preparedness
  • Coach, mentor, and support teams engaged in quality improvement (QI) initiatives across the organization, promoting systems thinking and the use of QI and patient safety best practices
  • Design and deliver QI and patient safety education to clinical and non-clinical interdisciplinary teams to build capacity, as required
  • Provide risk management support and advice as needed regarding high-risk situations, complex cases and patient safety matters
  • Engages staff, management, clients, families, partnership organizations, and other stakeholders in quality improvement initiatives as appropriate
  • Provides staff support to the Board level Quality Improvement & Safety Committee
  • Leads the corporate incident reporting process
  • Identifies and communicates potential risks associated with incident report trends to the leadership team, and supports the development of mitigation strategies
  • Proficient knowledge of legislation and regulatory environment applicable to the healthcare sector, including the Excellent Care for All Act, Quality of Care Information Protection Act and Privacy Act, Mental Health Act, PHIPA, Occupational Health and Safety Act, and Ontario Human Rights Code
  • Adheres to the organization’s Vision, Mission, and Values and its goals as outlined in its strategic plan
  • Represents CMHA York Region and South Simcoe in a positive and professional way with internal and external stakeholders

Requirements:

  • Bachelor's degree or diploma from a college or university
  • Regulated health professional with registration in good standing with the appropriate college preferred
  • Continuing education courses in quality improvement e.g. areas including root cause analysis, client safety, client engagement, client experience, quality improvement plans, dashboard development
  • Formal education in incident and risk management
  • Lean Six Sigma certification is an asset
  • Canadian Risk Management (CRM) designation an asset
  • Formal education in project management or PMP designation an asset

Prior Work Experience

  • Five years or more experience related to quality improvement, patient safety, and risk management, including experience developing quality scorecards and helping teams evaluate quality data
  • Experience facilitating the accreditation process
  • Experience in incident management
  • Experience in clinical risk management
  • Experience working in a community setting
  • Experience in project management an asset

Knowledge

  • Extensive knowledge of the accreditation process; preferably with Accreditation Canada
  • Thorough knowledge of the health care agenda in Ontario
  • Extensive knowledge of the issues, concepts and practices related to continuous quality improvement, patient safety and risk management Knowledge of client/family experience tools

Additional Requirements:

  • A valid Canadian Driver's license, a minimum of $1,000,000 third-party liability insurance

Working Conditions:

This role works extensively at a desk using a computer and telephone for up to 7 hours per day in a standard open office setting. Weekend and evening hours may be required.
