Organization:

SCAH-Sutter Care at Home - BayProvides operational leadership for a multidisciplinary home health team with a focus on infusion and specialty therapies. Oversees daily operations, staffing, scheduling, and resource management to ensure safe, efficient, and compliant delivery of patient care in alignment with regulatory and organizational standards.
Leads infusion nursing and liaison teams, supporting performance management, professional development, and clinical excellence. Coordinates complex infusion referrals, evaluates and approves home infusion therapies, and partners across disciplines to ensure high-quality, patient-centered care.
Drives standardization of workflows, policies, and best practices while monitoring clinical outcomes and key operational metrics. Ensures audit readiness, regulatory compliance, and effective documentation practices. Collaborates with internal and external stakeholders to optimize care delivery, support program growth, and resolve complex clinical or operational issues.
Ideal Candidate: Demonstrates proficiency with Epic and Microsoft applications, with strong hands-on nursing experience in critical care (ICU), oncology, or home infusion. Proven ability to effectively lead and manage remote teams, with strong communication, clinical judgment, and leadership skills.

EDUCATION:

  • Equivalent experience will be accepted in lieu of the required degree or diploma.
  • Bachelor's Nursing, or Health Science related to department



CERTIFICATION & LICENSURE:

  • RN-Registered Nurse of California
  • BLS-Basic Life Support Healthcare Provider

DEPARTMENT REQUIRED CERTIFICATIONS & LICENSURES

  • Department, Home Health & Hospice: AUTO-Automobile Insurance
  • Department, Home Health & Hospice: DL-Valid Drivers License - State Department of Motor Vehicles



TYPICAL EXPERIENCE:

  • 8 years recent relevant experience



SKILLS AND KNOWLEDGE:

  • Knowledge and expertise in clinical decision-making for the patient populations assigned, to meet the needs of the patients and to contribute to Quality Management review and evaluation.
  • Solid understanding of Home Health Nursing operations and workflows and how Home Health Nursing's process/workflow impacts other department operations
  • Demonstrated leadership skills, including team building, and coaching/mentoring with the ability to motivate and engage others.
  • Working knowledge and thorough understanding of state/federal/local and/or accreditation standards and requirements as it relations to operations and organization, with the ability interpret and implement applicable standards and requirements.
  • Demonstrates ability to develop and manage a budget.
  • Demonstrates tactical and analytical thinking and planning skills, and is able to develop plan to accomplish objectives, identify obstacles and resolve operational issues.
  • Ability to build and establish effective working partnership with employees, peers and/or leaders to achieve business objectives.
  • Verbal and written communication, and interpersonal skills.
  • Ability to handle difficult circumstances and make sound business decisions with little direction.
  • Demonstrates ability to work in a dynamic and fast-paced environment with changing business priorities.

Pay Range is $185,868.80 to $297,377.60 / annual salary

The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs. Base pay is only one component of Sutter Health’s comprehensive total rewards program. Eligible positions also include a comprehensive benefits package.
