Manager, Community Wellness Hubs

One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.
Reporting to the Director, Integrated Health and Community Programs, the Manager, Community Wellness Hubs provides leadership, operational oversight, and strategic coordination for Community Wellness Hub services located within Thunder Bay District Social Services Administration Board (TBDSSAB) housing sites and community outreach locations.
The position oversees the Interprofessional Team Members delivering mobile, community-based primary care, health promotion, chronic disease prevention, and social prescribing supports designed to improve access to care and reduce health inequities for underserved populations.
The position has a competitive salary, the Health Care of Ontario’s Pension Plan and an excellent group health benefits package.
The salary range for this position is $86,216 to $99,884 per year. We currently do not use AI to screen resumes.
QUALIFICATIONS:
• Master’s degree in a health care discipline such as Nursing, Social Work, Public Health, Health Administration or related field.
• Minimum three to five years of leadership or management experience in a community health or human services environment.
• Knowledge of community-based primary health care and the social determinants of health.
• Experience working with Indigenous, BIPOC, 2SLGBTQIA+, and culturally diverse populations.
• Demonstrated leadership experience within an interprofessional model of care.
• Experience with community engagement and partnership development.
• Knowledge of data collection, program evaluation and performance reporting.
• Experience using Electronic Medical Records systems.
• Valid Ontario Class “G” driver’s license and ability to travel to community outreach locations.
