Job Description

Reports to Director or Regional Director - Community Health. Support development of periodic Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP). Assist in developing and implementing key initiatives to successfully achieve goals of the CHIP. Represent Mercy in local collaborative efforts with community agencies and partners. Coordinate local community benefit data collection and reporting.
Monitor and track data and metrics that demonstrate the impact of community health initiatives. Oversee local Community Health Worker program, directly supervising CHWs in ED, clinic, and hospital. Manage and/or provide technical assistance of local grants (as needed) for community health services and patient assistance as applicable.

Position Details:

Qualifications:

  • Education: Bachelor’s degree in public or community health, healthcare, health education or related field required.
  • Experience: 2-3 years’ experience in community health or related field
  • Preferred Education: Master of Public Health (MPH) or Master of Healthcare Administration (MHA) preferred
  • Other: Must understand English both written and spoken, must be able to safely and successfully perform job-related functions, with or without reasonable accommodations required by federal, state, or local law, must be able to travel between Mercy communities.
