
Vancouver Coastal Health is seeking a Manager, Clinical Operations to lead the Personalized Support & Stabilization Team Plus (PSS+) and Extended Hours Nursing Program.
This role is ideal for a collaborative and systems-focused leader who is passionate about improving care for clients with complex needs and strengthening transitions from acute care to community services across Vancouver.
This position plays a key role not only in program operations, but also in supporting citywide access and flow for clients transitioning from hospital to community-based care.As a Manager, Clinical Operations with Vancouver Coastal Health, you will:• Provide clinical and operational leadership for the PSS+ program and Extended Hours Nursing Program across a multi-site, community-based environment. • Lead and support multidisciplinary teams delivering care to clients with complex needs, ensuring high-quality, client- and family-centred services. • Oversee day-to-day operations including staffing, service delivery, resource management, and performance monitoring. • Work collaboratively with acute care teams, community services, and system partners to improve transitions from hospital to home and community care. • Help identify and remove barriers impacting access and flow, particularly for complex and ALC clients, while maintaining awareness of system capacity across Home Health and Home Support services. • Lead operational planning, quality improvement initiatives, and program development aligned with organizational priorities. • Support and guide teams through change, service improvements, and evolving system needs. • Build strong relationships with internal and external partners to support coordinated, integrated care.
This role requires a leader who can balance program operations with a broader system perspective, working closely with local teams while contributing to solutions that improve care across the Vancouver community system.

QUALIFICATIONS:Qualifications:Baccalaureate (Masters preferred) in a relevant clinical professional area with seven (7) to ten (10) year's recent, related clinical and progressive leadership experience, including direct supervisory experience. Current registration/membership with a recognized professional association.
